Short-term rehabilitative care at Community Care Center provides physical, occupational and speech therapies to help clients recover and return to the highest level of functioning following injury, illness or surgery.
Our therapy team specializes in: Hip and knee replacement, Stroke recovery, Orthopedic injuries, Prosthetic/orthotic training, Strength building/fall reduction, Speech and swallow retraining.
In addition, we help clients transition from hospital to home by preparing them to manage their individual needs, providing patient and family education, and coordinating with their preferred home health agency for follow-up, in-home services and equipment.
